Symptoms of Acne

Acne is a condition of the skin, which causes spots, especially on the face, shoulders and upper back. Acne tends to begin at puberty, and although most sufferers are over it within a few years, it may carry on for life. Spots form in the glands of the skin that produce protective oils (sebaceous glands). The places where the spots usually occur are the face, shoulders, and upper back.

Acne may result in small spots which known as "blackheads" (or open comedones); Small spots which are white in colour, known as "whiteheads" (or closed comedones);These may become infected by a germ or bacterium (Propionibacterium acnes) which makes the spot inflamed, and results in a larger raised spot (papule), a spot filled with pus (pustule) or a hardened lump (nodule);The largest spots are more likely to leave lasting scars.

Acne Defined

Acne vulgaris (common acne) is a condition of the sebaceous glands of the skin. The sebaceous glands surround each hair follicle and produce sebum, an oily substance, to lubricate the hair. Acne usually appears on the face, back and chest.
Acne is the most common skin disease during adolescence and early adulthood. It peaks in females between the ages of 14 and 17 and in males between the ages of 16 and 19. Males are affected more severely than females. Eighty-five percent of high school students will have some acne. Ten percent of those with adolescent acne will continue to suffer from acne into their 20's and 30's. And sometimes "adult acne" will begin in the 20's, 30's and 40's and later years and continue due to stress and fatigue which can cause increased production of hormones by the adrenal gland with increased levels of androgens. These hormones worsen acne. Six percent of females and eight percent of males in their fifties and later still suffer from acne. Acne affects more than 17 million adult Americans and approximately 60 million adults in other countries.
Acne usually begins one to two years before puberty and is caused by stimulation of the facial sebaceous glands by androgens, the male hormones. (Women also produce small quantities of male hormones; this is the basis of acne in women.) It is thought that acne sufferers, rather than having higher levels of the hormones, actually have sebaceous glands that are more sensitive to the hormone.

The Occurrence of Acne

When your skin pores start to be clogged up with greasy, oily stuff, causing inflammation to occur, a skin condition known as acne results. If left untreated, acne could produce permanent scarring on one’s face, neck, or back. Acne is different from the common pimple that appears on the face, because acne will be inflamed and might turn out to be an infection. Acne can appear as whiteheads (closed pimples) or blackheads (open pimples), which then cause the distinguishing inflammation and possible infection.

When children begin to turn into adults, their levels of hormones causes changes in skin gland size, which causes more oil to be produced by the glands, thus often resulting in acne. The majority of acne is minor and vanishes at the end of teen years, but inflamed, infected acne can result in serious scarring.

Contrary to popular belief, there is no known study that shows particular foods, such as chocolate, cause acne. Proper diet and exercise will result in healthier skin, but will not necessarily prevent acne from appearing.

Mild forms of acne can be controlled by washing with a soft mild soap several times a day, and by refraining from picking at pimples while they are healing. There are several acne medications on the market that can control acne, but cosmetics and oil-filled lotions should be avoided when treating it.

Acne Statistics

Based on extensive research conducted by Bass & Boney, it has been found that acne commonly occurs in combination with other skin conditions. The complex interaction of multiple conditions can make acne even more difficult to treat and control.

Many skin care products when applied to the skin alter the natural pH balance of the skin making it susceptible to bacterial growth and infection. Upsetting the skin's natural balance can lead to yeast infections, perioral dermatitis, over production of sebaceous oils, and possibly worsening of a skin condition. Acne has been found to be a component in the complex interaction of symptoms occurring with rosacea. 65% of rosacea sufferers have reported a correlation between the onset of rosacea resulting from an acne occurrence. This may be the result of acne treatments that were too harsh and caused further damage to already sensitive skin leading to an exacerbation of rosacea in people who may have been pre-disposed to rosacea, or the acne treatment may have actually caused the rosacea. Many of the more commonly accepted products used to treat acne can actually cause further damage to the skin resulting in other skin conditions such as rosacea. Acne treatments are frequently used to open the pores (exfolients, various acid-based products, and retinoids) but can be very drying and irritating to the skin.

Over 70% of acne sufferers report having combination skin. The most frequent combination is skin that is oily in the T-zone of the face while the surrounding skin is either normal or dry. This combination skin is usually the result of over use of an acne treatment. When you dry the skin out too much with the use of acne drying agents, the skin goes into survival mode and produces more oil to protect the skin and body by forming a protective barrier between airborne bacteria and the body’s internal organs.

Controlling these conditions and the factors that cause and further irritate the skin is important in the management of acne. Factors which can play a role in this include but are not limited to: glycolic-based ingredients frequently found in foundation, anti-wrinkle creams and moisturizers, cleansers containing salicylic acid, alcohol, or benzyol peroxide. If it has the ability to dry or exfoliate, it can alter the natural balance of the skin. Once this balance is disturbed; the skin reacts with pimples, papules, and redness, itching, flaking and/or increased sensitivity.

Skin is the body’s largest organ and as such the first to show damage resulting from dehydration. Since body heat relates to vascular dilation (also referred to as flushing) in most people, the acne sufferer needs even more water than the average person. The body and skin dilates when it is acidic from 'acidic foods, drinks, and exercise (lactic acid). Those with undesirable skin conditions should consume between 10 and 16 glasses (4 liters) of cold water per day to cool the face and body and to assist the body in proper excretion of oil, "acidic" waste, (toxins). The hypothalamus is the body's thermostat, which regulates temperature, but it cannot achieve this objective without adequate water. The body is approximately 70% water while the eye is approximately 96% water. Water is needed for skin cell regeneration and skin tissue repair, efficient digestion, absorption, circulation and secretion. Water carries nutrients to the skin and body cells and removes waste from the cell and neutralizes or buffers the acids from exercise, stress, foods and drinks.

Seborrhea dermatitis occurs in approximately 35% of acne sufferers. While acne and seborrheic dermatitis are different skin disorders, sometimes they can coexist at the same time. Seborrheic dermatitis involves overactive sebaceous glands, which cause inflammation, flaking and a red rash in the central portion of the face. If one looks closely, the flakes usually have a greasy look, smell and feel. The dryness of seborrheic dermatitis is perceived because of the flaking, which consists of dried layers of accumulated oil. Seborrheic dermatitis may also result in yellowish scales, which develop on the scalp, the hairline and the eyebrows. This is often confused with the crusting and scaling on the eyelids that occur with ocular rosacea.

When treating acne, one sometimes sees an acne-like eruption around the mouth area. Known as peri-oral dermatitis, peri-oral refers to the facial area around the mouth while dermatitis pertains to inflammation, redness or irritation of the skin. In addition, there are usually small red bumps or even pus bumps and mild peeling as the skin is extremely aggravated. Peri-oral dermatitis symptoms characteristically involve the mouth area, but generally do not affect the lips themselves. You may also notice flaking of the skin at the site of occurrence. Many times if the flaking is isolated to the lip area it may be mistaken for chapped lips. Often the skin around the nose is affected too, and sometimes it can affect the area under and around the eyes. This condition may be wrongfully thought of as acne while others believe it to be a manifestation of rosacea. Peri-oral dermatitis is often aggravated by fluoridated or tartar-control toothpaste, chapstick, the ingredients in lipstick, and mouthwash. Discontinuing the use of fluoridated or tartar control toothpaste for six months may help reduce the symptoms of peri-oral dermatitis.

Understanding Moderate Stage Acne

In the moderate stage of acne, inflamed red lesions, large comedones and pustules with 0.4-0.8 inches width exist. These comedones and pustules can be viewed easily at the distance of more than 1 inch. Numerous whiteheads, blackheads, papules and pustules appear that cover an area from one quarter to three quarters of the face and/or other affected areas will also be present. Inflammatory or non-inflammatory acne lesions will be visible from a distance of 2.5m. Moderate acne can occur anywhere on the skin, that is, the face, arms, back, thighs, chest or shoulders.

Moderate acne consists of numerous lesions, such as blackheads, whiteheads or pustules, which appear at or near the surface of the skin. Blackheads result when a pore is only partially blocked, allowing some of the trapped sebum (oil), bacteria, and dead skin cells to slowly drain to the surface. The black color is not caused by dirt but is a reactionary process of the skin's own pigment, melanin, reacting with the oxygen in the air. Whiteheads result when a pore is completely blocked, trapping sebum (oil), bacteria, and dead skin cells, causing a white appearance on the surface. Acne Papules are small, red, tender bumps with no head. Do not squeeze a papule. It will do no good, and may exacerbate scarring. While acne pustules are similar to whiteheads, but are inflamed, and appear as a red circle with a white or yellow center.

Moderate acne progresses from mild acne when sufficient care is not taken, and needs to be handled before it escalates to severe acne. Dermatologists recommend early treatment for moderate acne because when moderate acne is not treated, it can progress to severe acne and permanent scaring can occur.

Traditional Acne Treatments May Cause Harm!

As adults, when we get a pimple we tend to treat it the same way we did as teenagers – our old acne zit cream or lotion packed with high concentrations of benzoyl peroxide, salicylic acid, and sometimes even topical vitamin A products.

The problem is that adult skin doesn't respond the same way to these products as teenage skin does. The result is red blotchy areas, more break outs and increased skin sensitivity.In trying to control this, we apply even more and stronger treatments to our skin but, instead of seeing an improvement, we actually see more damage and skin-related issues as the skin creates more oil to protect it from the damage and abuse of these harsh chemicals. Thus creating a vicious cycle of over-medicating the skin causing increased facial redness, clogged pores, and skin irritation.

To examine each of these traditional acne treatments more closer, we need to first examine what it is that we are actually applying to our skin.

Benzoyl peroxide is an oxygen-releasing chemical that causes drying, peeling, and antibacterial action on the skin. It is the #1 topical treatment for acne lesions. The release of oxygen is important to combat the anaerobic bacteria present in the acne-affected follicles. The peeling and drying effects peel the acne pustules (vesicles filled with pus) and open up the comedones (non-inflammatory acne lesions, such as black heads or white heads). Benzoyl peroxide is absorbed in the skin where it is metabolized to benzoic acid and then excreted as benzoate in the urine. Side effects consist mainly of skin irritation including: burning, blistering, crusting, itching, severe redness, and skin rash.

Salicylic Acid is another common ingredient in many acne products. Commonly known as aspirin, salicylic acid is also used for wart removal. Salicylic acid removes the outer layer of skin and is toxic in large amounts.

Retinols or Vitamin A based products act as a chemical peeling agent, which helps the skin to renew itself more rapidly. By increasing the rate of turnover of the skin, it reduces pimple and black head formation. It is commonly used to treat acne as well as to treat fine wrinkling.

Topical vitamin A has a number of common side effects--such as burning, redness, itching, peeling, and stinging; but, it is very effective at increasing the shedding of coenocytes, the cells of the topmost layer of the epidermis, contributing to smoother-feeling skin. It also reduces pigmentation and more evenly distributes pigment in the epidermis, resulting in the improvement of mottled hyper pigmentation. This really limits your ability to use these products as part of your daily skin care regimen.

TYPES OF ACNE - ACNE EXCORIEE

Acne excoriée is the term given for scratched or picked pimples. Acne excoriee is commonly called picker’s acne. Acne excoriée begins as a common acne outbreak of pimples. A pimple erupts, first one, then another. Many people find it almost impossible not to mess with, poke, prod, and squeeze the pimple. Acne excoriee is considered an emotional motivated or induced acne condition.

The problems that arise with picking and scratching acne blemishes is that it’s unsanitary and infection can set in, causing a bigger blemish. When the acne blemishes occur on the face of someone experiencing episodes of self-doubt, emotional turmoil, or low self-esteem, however, picking the blemishes can lead to a disturbing obsession that is self-perpetuating. An ongoing cycle of eruptions and picking and open sores begins that can be very difficult to stop.

Neurotic squeezing and pressing of the pimples, trying to rid the contents with the fingers, exacerbates minimal acne lesions on the face. As a result, hyperpigmentation and stellate contracted scars occur, while often the original underlying acne has healed. Acne excoriée is more common in females than males, and can be a sign of stress or depression. Sometimes it is just a bad habit that's hard to break; the acne may not actually be all that severe.

Acne excoriee usually begins during adolescence, when ideas of self-image are in upheaval and undergoing change, matching the hormonal and growth changes going on with the body. A fragile self-image may create an atmosphere where every imperfection perceived, real or not, is distortedout of porportion. By picking away at the acne blemishes on the face, many people find satisfaction, as if they are picking away at their troubles or their emotional pain. Others pick because they are nervous or uncomfortable with the changes in their body. They cause physical damage to their faces in order to fend off any physical advances they may be unperpared to deal with . Picking acne lesions may also be a coping mechanism designed to mask or hide the pain of physical or emotional abuse in the picker’s life. This trauma may be ongoing or an eent from their past.

One sign that distinguishes excoriated acne from acne vulgaris is by the presence of hyperpigmented brown spots on the face, chest, and other places where acne eruptions are likely. These brown spots are scars from previous blemishes that have been picked to the point of permanent tissue damage.

TYPES OF ACNE – ACNE COSMETICA

What you apply to your skin can be the CAUSE your acne. Acne cosmetica, or acne caused by cosmetics, is a common mild form of acne. Topical factors rather than hormones and acidic imbalances in the body trigger this type of acne. Even those people who are not susceptible to acne sometimes suffer from acne cosmetica. As the name implies, acne cosmetica is triggered by the use of cosmetics or skin care products such as deodorant soaps. Acne cosmetica usually occurs on the face, neck, hairline and scalp but can occur anywhere on the body.
Cosmetics, because of their chemical composition, are directly related to the occurrence of acne. In fact, because it is generated by outside chemical substances, acne cosmetica is easy to control and prevent. Acne cosmetica presents as small, rashy, pink bumps on the cheek, chin, forehead and even the scalp. The skin texture will appear and feel rough. There is little to no inflammation associated with this form of acne. It develops over a period of a few weeks or months and may be persistent for a long time. Discontinuing the use of the particular cosmetic, which triggers the onset, can contain the outbreak. Using cosmetics or other oily skin or hair products, which block pores. Acne cosmetica is the result of the use of comedogenic hair or skin care products. When a skin care product accumulates within the follicle, the pore becomes blocked. Excess skin oil builds up, clogging the pore and creating a blemish. Pomades or hair oils, heavy moisturizers, eye creams, and makeup are all common causes of acne cosmetica. Studies have shown that while make-up may not cause true acne, but it can worsen an already existing acne condition.
Identifying and isolating the acne-causing culprit is important. If the acne cosmetica occurs on the forehead, hairline or scalp, hair care products may be to blame. Bumpiness and breakouts over the entire face and neck area most likely are caused by moisturizer or foundation makeup. Once the source of the acne is identified and use is discontinued, acne cosmetica gradually improves.
Small bumps and whiteheads in the eye area or on the upper cheeks could be the result of eye makeup or creams. Many women find that their eye makeup can trigger or aggravate ocular symptoms that may mimic or cause ocular rosacea. Replace every three to four months the entire bottle of makeup, as there is a good chance of bacterial growth from the applicator after use on your eyelids to the eye make up bottle. Often when you experience a flare or worsening of ocular irritation, simply replacing your eye makeup can easily solve the problem as bacteria can reproduce rapidly in mascara, eyeliner or eye shadow containers. Each time you use an eye makeup product it is exposed to airborne bacteria and pollutants as well as bacteria that are always present on the skin's surface.

THE ACNE GROUP

Over use or abuse of acne treatments themselves can cause a worsening of acne. Makeup applied to the skin can in some instances cause conditions leading to the symptoms of acne. Diet, while not normally thought to be a cause of acne, can in fact lead to a pH imbalance in the body and the body's largest organ, the skin will also be affected. Hair spray, gels and conditioners can clog the pores along the hairline leading to acne pimples.

The best way to understand and treat acne is to first separate the myths from the facts.

1) 80 percent of the population suffers from acne.
2) Bacteria is generally the number one cause of acne.
3) There are many different types of acne.
4) Ultraviolet rays are acne contributors, not preventors. Sun rays damage skin, not help it. Although a tan may temporarily mask acne, the sun can make the skin dry and irritated, leading to more breakouts in the future. Always protect your skin by choosing a sunscreen of at least SPF (sun protection factor) 15 that says noncomedogenic or nonacnegenic on the label, which means it won't clog pores.
